May Hinala Pero Naisulat sa Logbook Bilang Fact: Ano ang Dapat Gawin ng Security Guard?

Last Updated = September 18, 2026

May nawawalang item sa premises.

May nagsabi:

“Baka yung contractor ang kumuha.”

Pagtingin sa logbook, nakasulat na:

“Contractor took the missing item.”

Kapag may hinala o allegation:

HUWAG GAWING FACT ANG HINDI PA VERIFIED.

Mas malinaw na sequence:

OBSERVE → IDENTIFY THE SOURCE → RECORD WHAT IS KNOWN → MARK WHAT IS UNVERIFIED → COORDINATE → UPDATE THROUGH THE APPROVED PROCESS

Halimbawa:

“At approximately 1430H, Mr. ___ reported that the item could no longer be located and stated that he suspected Mr. X. OIC ___ was informed at approximately 1435H. Matter remained for verification.”

Malinaw kung ano ang nangyari.

Malinaw din kung kanino galing ang hinala.

1. Unang Tanong: Ano Ba Talaga ang Alam Mo?

Bago magsulat, hatiin ang information.

Personally observed

Halimbawa:

“At approximately 1415H, I observed that the cabinet door was open.”

Reported information

Halimbawa:

“At approximately 1420H, Mr. Cruz reported that one handheld radio could not be accounted for.”

Verified information

Halimbawa:

“At approximately 1435H, OIC Reyes confirmed through the issued-equipment record that Radio No. 04 was assigned to Post 3.”

Unverified information

Halimbawa:

“A staff member stated that he believed the radio may have been taken by a contractor. This information had not yet been verified at the time of entry.”

Halimbawa:

Actual information:

“May nawawalang cellphone. Nakita raw si Employee A malapit sa area.”

Pero ang naisulat:

“Employee A stole cellphone.”

Halimbawa:

May pinto na bukas.

May gamit na nawawala.

May taong dumaan.

May CCTV.

  • sino ang kumuha;
  • bakit nawala;
  • kung theft ba talaga;
  • kung negligence ba;
  • kung may authorized movement ng property;

Halimbawa:

Mahina

“The visitor threatened the employee.”

Kung hindi mo iyon personal na narinig.

Mas malinaw

“At approximately 1710H, Ms. ___ reported that the visitor had threatened her.”

“I observed the visitor speaking in a raised voice near the lobby counter. I did not personally hear the complete conversation.”

  • clarification entry;
  • supplemental entry;
  • notation or correction;
  • supervisor acknowledgment;
  • separate incident report;
  • o ibang prescribed documentation.

Ang importanteng principle:

Preserve the integrity of the original duty record.

Halimbawa:

Original entry:

“1500H — Contractor took missing tool.”

Useful terms:

observed — personal mong nakita

reported — iniulat sa iyo

stated — sinabi ng isang identified source

confirmed / verified — na-confirm sa authorized source o procedure

could not be accounted for — hindi mahanap o ma-account sa oras na iyon

  • oras;
  • lugar;
  • observation;
  • action taken;
  • notifications;
  • at known status.

9. Hindi Objective ang Report Kapag Nilagyan Lang ng “According to Me”

Objective documentation is not about sounding formal.

Ang objective record ay malinaw kung:

ano ang nakita,

ano ang sinabi ng iba,

ano ang na-verify,

ano ang ginawa,

at

10. Ang Goal ng Logbook ay Continuity, Hindi Self-Protection Through Story Editing

Documentation protects the guard best when it is accurate—not when it tries to make everyone look good.

Kung may discrepancy, record and coordinate it according to procedure.

  • turnover;
  • supervisor review;
  • incident reconstruction;
  • follow-up action;
  • at accountability.

Tandaan

Kapag may incident:

OBSERVATION ≠ ASSUMPTION

REPORT ≠ VERIFIED FACT

SUSPICION ≠ CONCLUSION

Sundin ang authorized correction or supplemental-entry procedure.

CONTROL THE CONFLICT → RESTORE POST COVERAGE → NOTIFY / COORDINATE → PRESERVE FACTS → DOCUMENT → REVIEW THE CAUSE → PREVENT REPEAT

1. Unang Tanong: May Naiwang Security Gap Ba?

Ito ang unang operational review.

Hindi:

“Sino ang nagsimula?”

Kundi:

Halimbawa:

  • naiwan ba ang gate;
  • nawalan ba ng tao ang access-control point;
  • natigil ba ang CCTV monitoring;
  • walang nakasagot sa radio;
  • walang gumawa ng required roving;
  • may visitor o vehicle bang hindi na-verify;

Mahalaga ito dahil sa ilalim ng Republic Act No. 11917, kabilang sa private security services ang pagbabantay sa establishment o property, access control, pag-secure ng protected area, safety and protection of persons, at maintenance of peace and order within the area.

Practical questions:

May kailangang temporary replacement ba?

Sino ang mananatili sa critical post?

May pending visitor, vehicle, alarm, key, radio call, o turnover item bang kailangang i-endorse?

3. Verbal Argument at Physical Altercation: Magkaibang Level ng Risk

Hindi lahat ng argument ay pareho.

Kung verbal confrontation pa lang

Ang objective ay:

lower the tension → separate the parties → restore duty focus → notify the supervisor

4. Ang Supervisor Dapat Mag-review ng Operational Cause, Hindi Lang Personalidad

Madaling sabihin:

“Mainitin talaga ulo niyan.”

Professional discipline should also remain objective and focused on the conduct—not humiliation of the person.

5. Huwag Magdesisyon Base sa Pinakamaingay na Kuwento

Pagkatapos ng confrontation, madalas may kanya-kanyang version.

Guard A:

“Siya ang unang sumigaw.”

Guard B:

“Hindi, minura niya muna ako.”

Witness:

“Parang siya ang lumapit.”

CCTV:

Maaaring may video pero walang audio.

Kaya dapat paghiwalayin ang:

PERSONALLY OBSERVED

REPORTED BY GUARD A

REPORTED BY GUARD B

REPORTED BY WITNESS

SUPPORTED BY AVAILABLE RECORD

STILL UNVERIFIED

  • logbook entries;
  • CCTV footage;
  • radio communications;
  • post assignment or deployment record;
  • turnover/endorsement;
  • supervisor instructions;
  • witness information;
  • incident reports;
  • medical or injury record kung applicable;
  • at ibang existing authorized records.

7. Ano ang Dapat Ilagay sa Incident Report?

Hindi kailangan ng emotional wording.

Halimbawa:

Mahina

“Nagwala si Guard A at inaway si Guard B dahil may personal silang problema.”

Problema: may conclusion agad tungkol sa behavior at motive.

Mas factual

“At approximately 2015H, I observed S.G. A and S.G. B exchanging raised voices near Post 2. At approximately 2017H, I notified OIC ___ through radio. OIC ___ arrived at approximately 2020H and directed S.G. ___ to temporarily cover Post 2 while the involved guards were separated for further coordination.”

panatilihin ang sarili mong post kung kailangan → report the situation → request supervisor/backup → state only what you observed → follow authorized instruction

Personal friendship should not replace objective reporting.

At personal dislike should not turn into exaggerated reporting.

9. Ang Disciplinary Review ay Hiwalay sa Immediate Security Response

May dalawang magkaibang tanong:

Operational question

Na-control ba ang situation at naibalik ba ang security coverage?

Administrative or disciplinary question

May violation ba at ano ang proper agency/company response?

Huwag paghaluin.

The CSP Guard Force Administration framework emphasizes clear responsibility, supervision, objective discipline, communication, at coordinated performance of the security force.

May unclear o conflicting instruction ba?

May post-coverage problem ba?

May recurring unresolved interpersonal issue ba?

Tandaan

Kapag dalawang guard ang nag-away habang duty:

PERSONAL CONFLICT CAN BECOME A POST-COVERAGE PROBLEM.

Kaya ang professional sequence ay:

Control the conflict.

Protect the post.

Notify the supervisor.

Preserve the facts.

Document objectively.

Then review discipline and prevention.

REQUIREMENT → IDENTITY → AUTHORIZATION → ENTRY DECISION → COORDINATION → RECORDS → CORRECTIVE ACTION

  • malinaw ba ang access procedure;
  • updated ba ang authorization information;
  • available ba ang authorized verifier;
  • may special instruction bang hindi na-endorse;

  • valid ID;
  • approved visitor list;
  • visitor pass;
  • access card;
  • host o office confirmation;
  • work authorization;
  • appointment;
  • escort requirement; o
  • ibang prescribed verification.

Halimbawa:

  • kilalang contractor siya;
  • dating employee siya;
  • regular visitor siya;
  • may company ID siya;
  • may access card siya;

Ability to enter is not always authority to enter.

I-review:

  • Anong oras dumating ang tao?
  • Saang entrance siya pumasok?
  • Sino ang naka-duty?
  • Ano ang ipinakita niyang ID, pass, card, o document?
  • Ano ang tinanong o na-check?
  • May verification call ba?
  • Sino ang kinontak?
  • May instruction bang natanggap?
  • May exception bang pinayagan?

Supervision or procedure gap

  • visitor log;
  • guard logbook;
  • current access list;
  • visitor o contractor pass;
  • gate entry record;
  • access-card record;
  • turnover o endorsement;
  • relevant CCTV footage;
  • radio o control-room communication;
  • written instruction;
  • supervisor/client communication; at
  • incident report.

Preserve the original record.

CONTROL CURRENT RISK → NOTIFY → COORDINATE → VERIFY → FOLLOW THE APPROVED RESPONSE

  • kapkapan ang tao;
  • kumpiskahin ang gamit;
  • gumamit agad ng physical force;
  • i-detain; o
  • tratuhing criminal.

Personally observed

Ano ang personal mong nakita o ginawa?

Reported by another person

Ano ang sinabi lamang sa iyo?

Verified or confirmed

Halimbawa:

“At approximately 1420H, the individual entered through Gate 2 after presenting a company identification card. At approximately 1445H, the Administration Office informed the undersigned that no current visit authorization could be confirmed. The OIC was notified for further coordination.”

  • May after-hours verification procedure ba?

Kasama rito ang step-by-step guide, completed sample, Facts vs. Opinion at Source of Information guides, printable reference template, pre-submission checklist, at quick-reference card.

IDENTITY is not always AUTHORIZATION.

At sa mahusay na incident review:

FACTS muna bago CONCLUSION.

Mas protected ang guard, supervisor, agency, client, at site kapag malinaw ang verification, coordination, documentation, at corrective action.

Nawala ang Susi ng Restricted Area Habang Duty: Ano ang Dapat I-contain at I-document?

Last updated: September 14, 2026

Under RA 11917, private security services expressly include access control or denial and securing an area or property while protecting persons within that area.

REPORT → CONTROL ACCESS → VERIFY KEY STATUS → CHECK LAST KNOWN CUSTODY → PRESERVE RECORDS → DOCUMENT → FOLLOW AUTHORIZED SECURITY DECISION

Huwag muna i-frame as:

“Ninakaw ang susi.”

Kung ang verified fact lang ay:

hindi ito makita,

then the correct initial description is:

missing / unaccounted key.

At habang unresolved:

treat the situation as a potential access-control vulnerability according to the applicable site procedure.

1. I-report Agad—Huwag Hintayin ang End of Shift

Ito ang maling instinct:

“Baka nandito lang. Hanapin ko muna buong shift bago ko sabihin.”

The longer a restricted-area key remains unaccounted for, the longer na hindi malinaw kung sino ang posibleng may access.

A safer first move is:

notify the authorized supervisor or responsible key custodian immediately.

State only what is known:

“At approximately 1015H, I was unable to account for Restricted Area Key No. ___ during a custody check.”

Then include:

  • last personally verified possession;
  • expected storage or custody point;
  • current post/location;
  • immediate checks already completed; at
  • who was notified.

Do not wait for a confirmed unauthorized entry before escalating.

2. Control Access to the Restricted Area

The immediate security problem is not only:

“Nasaan ang susi?”

It is also:

“Secure pa ba ang area?”

Depending on the site SOP and supervisor instruction, practical containment may include:

  • restricting unnecessary entry;
  • assigning temporary observation or guard coverage;
  • confirming whether the affected door remains secured;
  • using another approved access-control measure;
  • suspending normal key use where authorized; or
  • routing legitimate access through an authorized custodian.

The exact response varies by site.

Do not improvise a lockout, rekeying, or access restriction beyond your authority.

The guard’s job is to recognize the exposure, protect the area within assigned responsibility, and escalate it.

3. Anong Key ang Nawawala?

Hindi lahat ng susi pareho ang risk.

Importanteng i-confirm:

  • ordinary room key ba;
  • restricted-office key;
  • storage-room key;
  • control-room key;
  • equipment-room key;
  • key for a critical door;
  • duplicate key;
  • master or higher-level access key; or
  • part ba ito ng larger key ring.

The broader the access provided by the missing key, the broader the potential exposure.

Pero huwag gawing public content ang exact keying system, master-key hierarchy, sensitive room identification, or exploitable access details.

The useful operational question is simply:

4. Verify the Last Known Custody

Build the narrowest factual timeline possible.

Ask:

May turnover ba?

May ibang authorized user ba?

May key log ba?

May signing-out procedure ba?

Example:

0800H — Key accepted during turnover.

0915H — Key used for authorized access.

0930H — Restricted area reportedly secured.

1015H — Key discovered unaccounted for.

That is already more useful than:

“Nawala sometime this morning.”

A tighter timeline helps determine which records should be checked and what area needs immediate protection.

5. Check the Key-Control Record

If the site maintains authorized key-control records, verify:

  • key number or identifier;
  • assigned custodian;
  • issuance time;
  • return time;
  • previous shift status;
  • authorized borrower/user;
  • duplicate status where applicable; at
  • unresolved discrepancy from prior turnover.

The point is not to fabricate a perfect paper trail after the problem happens.

The point is to know:

Ano talaga ang existing record?

If the record itself is incomplete:

document the gap honestly.

Do not add a fake earlier entry just to make the book look complete.

6. Preserve CCTV, Logs, and Access Records When Relevant

Kung may lawful and authorized records that can clarify the timeline, preserve them.

Possible sources:

  • CCTV;
  • key-control log;
  • post logbook;
  • visitor/contractor records;
  • access-control data;
  • duty roster;
  • turnover records;
  • radio log; at
  • supervisor inspection record.

Do not:

  • edit footage;
  • delete anything;
  • overwrite existing records;
  • backdate a key entry;
  • publicly circulate CCTV; or
  • personally access records outside your authority.

The professional value is preservation and objective review, not amateur investigation.

TSTS source controls specifically keep ordinary guard guidance within objective observation, coordination, documentation, and proper-authority turnover rather than sensitive investigation tactics.

7. Missing Key ≠ Confirmed Theft

Possible explanations include:

  • misplaced;
  • accidentally retained by an authorized person;
  • left at an authorized location;
  • incorrect previous turnover;
  • lost elsewhere;
  • taken without authority; or
  • deliberately stolen.

Until facts establish which one happened:

do not convert a missing key into confirmed theft.

The incident report should clearly distinguish:

Observed:
Key cannot currently be accounted for.

Reported:
Another employee says they saw someone holding a key.

Verified:
Confirmed through authorized records or proper investigation.

That distinction protects both the investigation and the people involved.

8. Kapag Nahanap ang Key, Tapos Na Ba?

Not necessarily.

Suppose the key is found one hour later under a desk.

Important questions remain:

Was it continuously under authorized control?

Could somebody else have accessed it?

Was the restricted area entered during the missing period?

May security-control weakness bang na-expose?

Depending on risk and site policy, authorized management may still decide to:

  • review access records;
  • inspect the restricted area;
  • check whether anything was disturbed;
  • change the key-control process;
  • reassess custody;
  • replace or reconfigure the lock; or
  • take another security measure.

That decision should come from the authorized client/agency/site management—not from an ordinary guard acting alone.

9. Kapag Master or High-Access Key ang Missing

Mas serious ang exposure kapag isang key can open multiple controlled areas.

Pero the public lesson should stay high-level:

broader access capability = potentially broader containment and management review.

Do not publicly disclose:

  • which doors share a master key;
  • exact access hierarchy;
  • number of duplicate master keys;
  • key codes;
  • lock configurations; or
  • exploitable building access patterns.

The guard should report the key’s authorized classification to the responsible supervisor/custodian and let management determine the appropriate protective action.

10. Ano ang Dapat Nasa Incident Report?

A clean report can follow this structure:

Discovery

When was the key first found unaccounted for?

Key identification

Use only the approved identifying detail necessary for the report.

Last personally verified custody

When and where was it last personally confirmed?

Current condition

Is the restricted area still physically secured?

Immediate action

What access-control or protective step was taken within the guard’s authority?

Notification

Who was informed, when, and how?

Records preserved

What log, CCTV, access, or turnover information was identified or preserved?

Instructions received

What did the supervisor or responsible authority direct?

Final status

Recovered? Still missing? Custody transferred? Further review pending?

Example:

“At approximately 1015H, Restricted Area Key No. ___ could not be accounted for during a custody check. The key was last personally verified at approximately 0915H. The area remained secured at the time of discovery. Supervisor ___ was notified at 1018H, and temporary access control was implemented as instructed.”

That is stronger than:

“May nagnakaw ng susi.”

when nobody has established that yet.

11. Review the System, Not Only the Person

If a key goes missing, possible individual accountability should be reviewed.

But also ask:

May proper key-control system ba?

May designated custodian ba?

May turnover verification ba?

May secure authorized storage ba?

Na-audit ba regularly ang keys?

May recurring discrepancy bang hindi ina-address?

Professional loss prevention is fundamentally preventive. The CSP Manual emphasizes moving from reactive apprehension toward preventing loss and reducing opportunities for loss.

The bigger lesson is:

A good key-control system should make it difficult for a key to disappear unnoticed.

12. Huwag Maghintay ng Unauthorized Entry Bago Ayusin ang Gap

Sometimes the response is:

“Wala namang pumasok, so okay lang.”

That misses the security lesson.

If a restricted-area key became unaccounted for, an access-control vulnerability already existed—even if no actual unauthorized entry is later proven.

Loss prevention aims to reduce the opportunity before a bigger incident occurs.

So after the immediate incident:

review why the key became unaccounted for and close the gap.

Practical First-Move Checklist

Kapag nawawala ang restricted-area key:

1. CONFIRM
Check whether the key is genuinely unaccounted for.

2. REPORT
Notify the authorized supervisor/key custodian promptly.

3. CONTAIN
Protect the restricted area according to approved procedure.

4. TRACE
Establish the last verified custody and relevant timeline.

5. PRESERVE
Protect logs, CCTV, access records, and turnover information.

6. DOCUMENT
Record facts, notifications, actions, and current status.

7. FOLLOW THROUGH
Carry out the authorized corrective or access-control instruction.

Tandaan

The key question is not merely:

“Nasaan ang susi?”

It is:

“Habang nawawala ang susi, protected pa ba ang restricted area?”

So when a restricted-area key cannot be accounted for:

Don’t hide it.
Don’t guess.
Don’t accuse.
Don’t backdate.

Instead:

REPORT → CONTROL ACCESS → VERIFY CUSTODY → PRESERVE RECORDS → DOCUMENT → LET AUTHORIZED MANAGEMENT CLOSE THE ACCESS GAP

Because a missing key can be replaced.

A compromised access-control system is the bigger problem.

Actual key-loss and restricted-area procedures depend on:

  • approved post orders;
  • site SOP;
  • key-control policy;
  • Area of Responsibility;
  • PSA policy;
  • client policy;
  • security plan;
  • lock/access-control configuration;
