May Hinala Pero Naisulat sa Logbook Bilang Fact: Ano ang Dapat Gawin ng Security Guard?

Last Updated = September 18, 2026

May nawawalang item sa premises.

May nagsabi:

“Baka yung contractor ang kumuha.”

Pagtingin sa logbook, nakasulat na:

“Contractor took the missing item.”

Kapag may hinala o allegation:

HUWAG GAWING FACT ANG HINDI PA VERIFIED.

Mas malinaw na sequence:

OBSERVE → IDENTIFY THE SOURCE → RECORD WHAT IS KNOWN → MARK WHAT IS UNVERIFIED → COORDINATE → UPDATE THROUGH THE APPROVED PROCESS

Halimbawa:

“At approximately 1430H, Mr. ___ reported that the item could no longer be located and stated that he suspected Mr. X. OIC ___ was informed at approximately 1435H. Matter remained for verification.”

Malinaw kung ano ang nangyari.

Malinaw din kung kanino galing ang hinala.

1. Unang Tanong: Ano Ba Talaga ang Alam Mo?

Bago magsulat, hatiin ang information.

Personally observed

Halimbawa:

“At approximately 1415H, I observed that the cabinet door was open.”

Reported information

Halimbawa:

“At approximately 1420H, Mr. Cruz reported that one handheld radio could not be accounted for.”

Verified information

Halimbawa:

“At approximately 1435H, OIC Reyes confirmed through the issued-equipment record that Radio No. 04 was assigned to Post 3.”

Unverified information

Halimbawa:

“A staff member stated that he believed the radio may have been taken by a contractor. This information had not yet been verified at the time of entry.”

Halimbawa:

Actual information:

“May nawawalang cellphone. Nakita raw si Employee A malapit sa area.”

Pero ang naisulat:

“Employee A stole cellphone.”

Halimbawa:

May pinto na bukas.

May gamit na nawawala.

May taong dumaan.

May CCTV.

  • sino ang kumuha;
  • bakit nawala;
  • kung theft ba talaga;
  • kung negligence ba;
  • kung may authorized movement ng property;

Halimbawa:

Mahina

“The visitor threatened the employee.”

Kung hindi mo iyon personal na narinig.

Mas malinaw

“At approximately 1710H, Ms. ___ reported that the visitor had threatened her.”

“I observed the visitor speaking in a raised voice near the lobby counter. I did not personally hear the complete conversation.”

  • clarification entry;
  • supplemental entry;
  • notation or correction;
  • supervisor acknowledgment;
  • separate incident report;
  • o ibang prescribed documentation.

Ang importanteng principle:

Preserve the integrity of the original duty record.

Halimbawa:

Original entry:

“1500H — Contractor took missing tool.”

Useful terms:

observed — personal mong nakita

reported — iniulat sa iyo

stated — sinabi ng isang identified source

confirmed / verified — na-confirm sa authorized source o procedure

could not be accounted for — hindi mahanap o ma-account sa oras na iyon

  • oras;
  • lugar;
  • observation;
  • action taken;
  • notifications;
  • at known status.

9. Hindi Objective ang Report Kapag Nilagyan Lang ng “According to Me”

Objective documentation is not about sounding formal.

Ang objective record ay malinaw kung:

ano ang nakita,

ano ang sinabi ng iba,

ano ang na-verify,

ano ang ginawa,

at

10. Ang Goal ng Logbook ay Continuity, Hindi Self-Protection Through Story Editing

Documentation protects the guard best when it is accurate—not when it tries to make everyone look good.

Kung may discrepancy, record and coordinate it according to procedure.

  • turnover;
  • supervisor review;
  • incident reconstruction;
  • follow-up action;
  • at accountability.

Tandaan

Kapag may incident:

OBSERVATION ≠ ASSUMPTION

REPORT ≠ VERIFIED FACT

SUSPICION ≠ CONCLUSION

Sundin ang authorized correction or supplemental-entry procedure.

I-document kung ano ang bagong na-verify.

Nawala ang Service Firearm Habang Duty: Ano ang Dapat I-report, I-secure at I-document?

Last updated: September 13, 2026

Mas safe ang first response:

REPORT → SECURE → VERIFY → PRESERVE → DOCUMENT → TURN OVER

Under RA 11917, agency firearms are regulated firearms of the PSA, and possession by the PSP is generally tied to the guard’s tour of duty, proper uniform, assigned establishment, and applicable firearm authorization. PSA firearms are also subject to PNP inventory inspection.

Mabilis na Sagot

Kapag napansing nawawala ang service firearm:

1. I-report agad sa authorized supervisor/agency chain.
3. I-verify ang firearm issuance at turnover records.
4. I-preserve ang relevant CCTV, logs, at communication records.
5. I-document ang exact timeline at actions taken.
6. I-coordinate ang required police at PNP/FEO/SOSIA/RCSU process through the proper authorized agency personnel.

1. I-report Agad—Huwag Munang “Ayusin Nang Tahimik”

May malaking difference sa:

“Nawawala ang firearm.”

at

Report what is verified:

“Hindi na ma-account ang issued service firearm as of approximately 02:15.”

Then state:

  • last known verified status;
  • saan ito dapat naka-custody;
  • kailan huling nakita o na-account;
  • sino ang na-notify; at
  • anong immediate action ang ginawa.

RA 10591 expressly treats a lost or stolen firearm as a loose firearm, kaya serious ang pagkawala nito.

2. I-secure ang Immediate Area—Pero Huwag Gawing Personal Manhunt

Kung bagong discovery ang pagkawala at safe gawin within assigned duty:

  • control unnecessary access;
  • preserve the immediate area;
  • keep unauthorized persons away from relevant records or possible incident location; at
  • follow the supervisor’s or proper authority’s instructions.

Pero hindi ibig sabihin na puwedeng basta:

  • mag-search ng personal belongings nang walang authority;
  • mag-detain ng tao dahil lang suspicious;
  • mag-interrogate;

TSTS controlled-investigation guidance keeps the ordinary guard’s public role at safety, objective observation, coordination, record protection, and proper-authority turnover.

3. I-verify ang Firearm Accountability Record

Isa sa unang kailangang malinaw:

Anong firearm mismo ang nawawala?

Check only the authorized records available to the responsible personnel:

  • firearm description;
  • serial/registration information;
  • name of assigned PSP;
  • Duty Detail Order or equivalent authorized issuance record;
  • time/date of issuance;
  • post or assignment;
  • previous turnover; at
  • authorized firearm custodian where applicable.

The 2022 IRR of RA 11917 requires the DDO to identify the assigned PSP, firearm description, location/duration of assignment, and authorized bonded firearm custodian. The current IRR is officially registered with UP Law Center’s ONAR.

4. Preserve CCTV at Existing Records

Kung may CCTV coverage, logbook, electronic access record, radio log, visitor record, o supervisor inspection record:

preserve—do not manipulate.

Mas useful malaman:

  • anong time window ang relevant;
  • anong camera ang posibleng may useful footage;
  • sino ang authorized CCTV custodian;

Professional security practice treats records and loss history as important inputs in identifying vulnerabilities and designing corrective controls.

5. Ano ang Dapat Nasa Incident Report?

Dito madalas nasisira ang protection ng guard.

Huwag puro conclusion.

Personally observed

Example:

“At approximately 0215H, I conducted an accountability check and observed that the issued firearm was no longer at its expected custody point.”

Last verified status

“The firearm was last personally confirmed in my custody at approximately ___.”

Kung hindi personally confirmed, huwag kunwari confirmed.

Reported information

Kung may ibang nagsabi:

“S.G. ___ reported that…”

Hindi:

“Ganito talaga ang nangyari.”

Notifications

Record:

  • name/designation;
  • time notified;
  • method of notification; at
  • instruction received.

Actions taken

Example:

  • access controlled;
  • supervisor informed;
  • CCTV preservation requested;
  • issuance record checked;
  • police/authorized authority contacted; at
  • turnover completed.

Final status at time of report

Kung hindi pa recovered:

“Firearm remained unaccounted for at the time this report was submitted.”

Hindi kailangan mag-imbento ng ending.

Documentation should distinguish observation, report, suspicion, and confirmation. Iyan ang permanent TSTS protection layer for incident reporting.

6. Sino ang Magre-report sa PNP/FEO?

Mahalagang distinction ito.

Ang ordinary guard should immediately notify the authorized agency/supervisor chain according to the applicable procedure.

Under Section 40 of RA 10591, failure of the licensed firearm holder to report a lost or stolen firearm to the PNP Firearms and Explosives Office within 30 days from discovery carries a ₱10,000 fine.

The 2018 Revised IRR further states that for a juridical entity, written notification may be made by an authorized officer or representative and accompanied by the applicable affidavit of loss and local-police certification reflected in the police blotter.

Pero tandaan:

Ang 30 days ay statutory deadline—not a recommended waiting period.

Operationally, a missing service firearm should be escalated promptly, because every hour na hindi accounted ang firearm may increase risk.

The RA 11917 IRR also identifies loss, theft, or taking of a PSA/CGF/GSF-issued firearm as a type of incident that may trigger SOSIA/RCSU motu proprio investigation.

7. Kapag Na-recover ang Firearm, Tapos Na Ba?

Hindi agad.

Record:

  • saan na-recover;
  • kailan;
  • sino ang nakakita o nag-turn over;
  • kanino na-turn over;
  • anong condition ang observed without unnecessary handling; at
  • anong further instruction ang ibinigay.

Then allow the authorized agency and proper authorities to determine what additional examination, investigation, or regulatory action is required.

Do not simply return it to normal circulation and pretend nothing happened.

Huwag Gawin ang Apat na Ito

Huwag itago ang pagkawala.

Delayed concealment can make accountability and recovery harder.

Huwag agad magbintang.

Missing does not automatically mean a specific person stole it.

Huwag baguhin ang records.

A clean-looking false record is more dangerous than an honest record showing a gap.

Huwag gumawa ng sariling investigation beyond authority.

Protect the information and cooperate with the authorized investigation.

Tandaan

Kailan huling verified?
Sino ang kailangang ma-notify ngayon?
Ano ang dapat i-secure?
Anong records ang kailangang i-preserve?
At ano ang maayos na factual turnover?

Sa ganitong incident:

FAST REPORTING protects recovery.
CLEAR RECORDS protect accountability.
PROPER COORDINATION protects the investigation.

At pinakamahalaga:

May nadapa.
May nag-away.
May nawawalang gamit.
May suspicious person.
May found property.

Emergency response and safety first.
Then access control.
Then coordination.
Then record.
Then report.

saan nangyari,
anong oras nakita,
sino ang involved or present,
sino ang naabisuhan,
anong action ang ginawa,
ano ang na-turn over,
at ano ang na-report according to SOP.

“Linisin muna natin.”
“Itabi muna yung gamit.”

Emergency Response and Safety Still Come First

Safety comes first.

Kung kailangan tumawag ng supervisor, control room, medical responder, barangay, PNP, BFP, or proper authority through approved channel, i-coordinate according to procedure.
Kung may crowd risk, control access kung within duty instruction and safe to do.
Kung may danger sa area, secure people first.

After safety and emergency action, huwag gumawa ng unnecessary movement, cleaning, recording, or handling na puwedeng magpalabo ng documentation.

“Linisin muna.”
“Ayusin muna bago makita ng iba.”
“Tabi muna natin para hindi pangit tingnan.”
“Walisin muna para malinis.”

Sa normal maintenance concern, okay ang cleaning.

Pero kapag incident scene na may injury, damage, missing item, commotion, found property, or possible complaint, kailangan munang mag-ingat.

Bakit?

Halimbawa:

Kapag may incident, maraming curious.

May gustong makakita.
May gustong magtanong.
May gustong mag-picture.
May gustong makialam.

authorized personnel,
supervisor or OIC,
medical responder if needed,
client representative if authorized,
maintenance if needed and instructed,
proper authority if required,
or personnel covered by site procedure.

CCTV Preservation: I-flag Agad Through Proper Channel

Example:

“Possible CCTV coverage near parking B, camera facing delivery gate. Please preserve or review according to procedure.”

Bakit kailangan early?

Dahil may sites na may overwrite period.
May camera na limited ang storage.
May system na hindi agad accessible.
May kailangan munang authorized request.
May privacy and access protocol.

Hindi dapat basta:

kumuha ng footage sa personal device,
i-send sa group chat kung hindi authorized,
i-post online,
i-share sa hindi involved,
o magbigay ng copy without proper approval.

Ang guard’s safer role is to flag, coordinate, and document that CCTV preservation or review was requested through proper channel.

Witness Information: Huwag Hayaan Maghalo-Halo agad ang Stories

Sa incident, may mga nakakita.

Pero may risk kapag lahat agad nag-usap-usap.

“Ganito daw nangyari.”
“Sabi niya ganito.”

Puwedeng i-record according to SOP:

name or identifier if allowed,
contact or department if required and authorized,
location of witness,
what they directly observed,
time they observed it,
and who received the information.

Pero iwasan ang leading questions, pressure, or sariling conclusion.

Mas safe ang factual:

“Ano po ang nakita ninyo?”
“Saan po kayo nakapuwesto noong nangyari?”
“Anong oras po roughly?”
“Sino po ang unang naabisuhan?”

Hindi safe ang:

“Siya ang may kasalanan, di ba?”
“Nakita mo siyang gumawa, tama?”
“Sabihin mo ganito para malinaw.”

Ang goal ay clarity, hindi manipulation.

Found Property: Huwag Informal ang Turnover

May wallet na nakita.
May cellphone na naiwan.
May ID na napulot.
May susi.
May bag.
May gamit na naiwan sa lobby or parking.

Common mistake:

“Guard, ikaw muna humawak.”
“Tago mo muna sa drawer.”
“Bigay mo na lang kapag may nag-claim.”
“Picture-an mo muna.”
“Buksan natin para malaman kung kanino.”

Found property should not be handled casually.

Depende sa site SOP, dapat may proper receiving and turnover process.

Mas protected kung malinaw:

saan nakita,
anong oras nakita,
sino ang nakakita,
sino ang tumanggap,
description ng item without unnecessary exposure,
condition ng item,
turnover record,
claiming procedure,
and authorized receiving person.

Huwag basta buksan, gamitin, iuwi, i-post, or i-turn over informally.

Kung kailangan i-secure temporarily, gawin only according to site procedure and proper receiving process.

Cellphone Recording: Hindi Automatic na Safe Move

May incident, tapos instinct ng iba:

“Kuhanan mo ng video.”
“I-record mo para may proof.”
“Live mo.”
“I-send mo sa GC.”

Delikado ito.

Hindi dahil bawal lagi ang documentation.

Kundi dahil personal cellphone recording can create issues:

privacy issue,
authorization issue,
chain of information problem,
duty-priority distraction,
uncontrolled sharing,
wrong interpretation,
or conflict with client/site policy.

Kung may CCTV, radio log, logbook, supervisor report, incident form, or authorized documentation channel, iyon ang safer path.

Kung may instruction from authorized personnel or emergency need under site procedure, follow the approved process.

Pero as general rule, huwag gawing default na personal cellphone ang first documentation tool ng guard.

Your first job is not to become a content recorder.

Your first job is safety, coordination, access control if needed, and proper reporting.

Timestamp Mismatch: Bakit Nakakalito sa Review

May incident review.

CCTV time says 8:42 PM.
Logbook says 8:47 PM.
Radio record says 8:45 PM.
Guard statement says around 8:40 PM.

Kung exact time, state exact time.
Kung approximate, use “around” or “approximately.”
Kung delayed ang entry, indicate when the incident was observed and when it was recorded.
Kung may CCTV time discrepancy, flag it through proper channel.

Example:

“Incident observed at around 8:40 PM. Radio report made shortly after. Logbook entry written at 8:52 PM after initial response.”

Mas malinaw iyon kaysa maglagay ng isang time na parang lahat nangyari doon.

Bakit Critical ang First Five Minutes

Sa first five minutes ng incident, maraming nangyayari.

May papasok.
May aalis.
May magtatanong.
May magpi-picture.
May gagalaw ng gamit.
May maglilinis.
May magbibigay ng opinion.
May magsasabing “ako na bahala.”
May witness na kakausapin ng ibang tao.
May CCTV na dapat ma-flag early.
May radio report na dapat malinaw.

Dito puwedeng lumiwanag or lumabo ang buong incident review.

Kaya sa first five minutes, practical sequence:

Safety and emergency response first.
Control access if needed and if within site procedure.
Avoid unnecessary movement, cleaning, or disturbance of the scene.
Coordinate with OIC, supervisor, control room, or authorized personnel.
Flag CCTV preservation or relevant records through authorized channel.
Handle witness initial accounts according to established procedure.
Turn over found property or relevant items through proper receiving process.
Record time, location, persons involved, actions taken, and turnover.
Report or escalate according to site SOP, agency/client instruction, and proper authority guidance.

Hindi kailangang perfect.

Pero kailangang may sequence.

What Should the Guard Record After the Situation Is Controlled?

After safety and initial coordination, kailangan ng factual record according to site procedure.

Possible details:

date and time observed,
exact location,
who reported or discovered the incident,
persons involved if known and allowed to record,
witnesses or persons present if required,
initial condition observed,
safety action taken,
radio report or notification made,
who acknowledged or received the report,
supervisor/OIC/control room notified,
CCTV or record preservation flagged,
found property or item turnover details,
arrival of authorized personnel,
and follow-up instruction received.

Huwag maglagay ng conclusion kung hindi verified.

Mas safe:

“Male person observed near restricted door, no visible ID.”

Hindi agad:

“Magnanakaw.”

Mas safe:

“Wallet found at lobby sofa area and turned over to front desk supervisor with record.”

Hindi:

“Nakuha ko yung wallet.”

Documentation protects better when it is factual, specific, and within procedure.

Common Mistakes sa Incident Scene Handling

1. Linis agad bago ma-coordinate

2. Galaw agad ng gamit
Kung walang safety reason or instruction, iwasan ang unnecessary movement.

3. Pinapasok ang maraming tao
Kapag maraming pumapasok, lumalabo kung sino ang involved, witness, or unauthorized presence.

4. Pinag-uusap agad ang witnesses
Puwedeng maghalo ang stories bago makuha ang initial account.

5. Personal cellphone ang ginawang default documentation
May privacy, authorization, sharing, and duty-priority issues.

7. Walang turnover record sa found property

Mas malinaw kung hiwalay ang observed time, report time, response time, and logbook entry time.

9. Gumawa ng sariling conclusion
Mas protected ang factual observation kaysa accusation.

Safety.
Coordination.
Control.
Record.
Report.

Kung may immediate danger, respond according to emergency procedure.

Tama ba ang first action?
Na-coordinate ba agad?
Na-control ba ang access?
Na-preserve ba ang relevant records through proper channel?
May clear turnover ba?
May factual documentation ba?
May proper report ba?

Hindi ito para takutin ang guard.

Ito ay protection.

Mas malinaw ang sequence, mas protected ang guard.

Key Takeaways

Emergency response and safety come first.
Control access if needed and if within site procedure.
Flag CCTV preservation early through authorized channel.
Do not rely on personal cellphone recording unless authorized by procedure.
Handle found property through proper receiving and turnover process.
Witness information should be handled according to established procedure.
Timestamp mismatch should be clarified, not guessed.
The first five minutes can affect the whole incident review.
Clear coordination and factual documentation protect the guard.

FAQ

Kung may safety risk, emergency, injured person, fire, or immediate danger, emergency response comes first. Ang iwasan ay unnecessary movement, cleaning, or handling kapag wala pang coordination or instruction.

Depende sa situation and SOP.

Huwag gawing default. Personal cellphone recording may create privacy, authorization, sharing, and duty-priority issues. Follow site procedure and authorized documentation channels.

I-flag agad sa OIC, supervisor, control room, or authorized personnel. Do not personally copy, share, or distribute CCTV unless authorized by procedure.

Ano ang dapat gawin kapag may found property?

I-turn over through proper receiving process. Record where it was found, time, item description, who received it, and claim procedure if required by site SOP.

Bakit importante ang witness information?

Witness information can help clarify timeline. Pero dapat handled properly. Iwasan ang pressure, leading questions, or letting stories mix before initial account if site procedure requires documentation.

Ano ang dapat isulat ng guard after incident?

Record factual details: time, location, what was observed, who was notified, who acknowledged, safety action taken, turnover, CCTV flagging, and supervisor instruction.
